Hello CMS Athenas and Stags! We are excited to return to action for the 2026-27 academic year. There are items to complete over the summer before your arrival on campus and participation in CMS Athletics. Below is a checklist of the items and the due dates of when to complete the requirements.
Each department will send you emails with more detailed information. If you have any questions regarding any of these items, please utilize the email addresses in the checklist for a quick response.

Medical Clearance
The appropriate medical paperwork is REQUIRED to be cleared to practice and to compete.
New Athlete Due Date: July 15, 2026
Returning Athlete Due Date: August 1, 2026
If your paperwork is not completed by this date, there may be a delay in when you can start practicing with your team.
Physicals: Athletic Pre-Participation Physical Exam (PPE): All student-athletes must undergo an annual comprehensive medical examination by a qualified physician (MD or DO) no earlier than 3 months before the beginning of the first sport of participation of an academic year. Student-athletes must submit documentation of a current athletic physical in ATS (instructions below). The PPE form can be found here: PPE Form
Athlete Portal (ATS): Before participating in athletic activity on a team for CMS, student-athletes must submit the following to the Sports Medicine Department through ATS: their current address, emergency contact, insurance information, health history information, sickle-cell test results, and the pre-participation physical. Sway Baseline Concussion Assessment: All athletes are required to complete a baseline concussion assessment through the Sway Medical App prior to the start of each sports season. This requirement applies annually and before the start of every season for multi-sport athletes. Those participating in multiple sports should use the Sway code associated with the sport that occurs first in the year (e.g., if you participate in fall and spring sports, athletes should use the fall sport code). Compliance Clearance
Compliance paperwork is REQUIRED to participate in ANY team activities. Mid-July, you will receive an email when NCAA Eligibility and CMS Forms are available to complete. They are due August 1.
- New student-athletes: If you did not already during the recruiting process, register with the NCAA Eligibility Center. Create a free Profile Page Account (Option 3) through the NCAA EC Website.
- New AND returning student-athletes: Complete the 8 NCAA Eligibility and CMS Forms. Information about the forms and instructions to complete them can be found here: Eligibility Form Instructions
Equipment Room

Learn about the equipment room and submit your sizes on the sizing chart on the Equipment Room site.
